STUDY PERMIT CHECKLIST 2010- GENERAL DOCUMENTS CHECKLIST

SET I

Checklist – Ensure you attach this checklist as a cover sheet on your application

1. Current Passport
2. Application form (two recent, original passport-sized photos to be attached to form.)
3. Personal Information Form: Student Questionnaire (please use questionnaire attached to this message until the questionnaire is up-dated on the web-site)
4. Letter of acceptance from a Canadian educational institution showing planned start and end date of program and any conditions upon acceptance into program
5. Use of a Representative form (IMM 5476)
6. For Quebec-bound students only – Copy of Certificat d’acceptation du Québec-CAQ

Items in bold print below are STRONGLY RECOMMENDED

SET II

1. IELTS test results taken within last 24 months with overall band not less than 6.0
2. Copy of your most recent diploma/ leaving certificate, and most recent mark sheets
3. Other educational certificates/diplomas/degrees (including mark sheets)

SET III

1. Copy of all receipts for payment provided by the Canadian educational institution (pre-payment of first term is recommended)
2. Copy of an Educational Loan from an Indian Chartered Bank equivalent to at least 80% of tuition +living + travel expenses for one academic year (normally, first year’s tuition unless already paid, plus $8000). See information on the next page for the list of acceptable banks.
3. Form 16 for salaried employees or income tax return document
4. Bank record showing transaction history for: current month, as well as bank record from 6 months ago, and from 1 year ago for the same bank account

Photographs

Please provide two identical copies of a recent photograph of yourself. Please take following precautions while submitting the photographs

The photographs should be -

1. In Colour
2. Full Face (Without sunglasses and normally without a hat or other head covering, unless the applicant wears such items because of their religious belief or ethnic background)
3. Taken against a light (white or light blue) background so that the features are distinguishable and contrasted against the background.
4. Clear and of good quality
5. Printed on normal photographic paper
6. Unmounted.
7. 2 inches x 1.5 to 2 inches in size
8. Write your Name and Date of Birth on the back of your photographs
9. Scanned and Digital Photographs are NOT accepted.
